Additional Membership Terms:

Additional units may be purchased during your visit at a discounted rate of $11 per unit.
Botox will be administered by a Physician.
Membership Renewal after 1-year, the patient will be charged the yearly enrollment fee of $350 on the 13th month and will continue with 11 monthly payments of $99.
The $99 monthly fee will recur monthly on an ongoing basis until the end of the year. Please contact our office for assistance.
